Case 1: This is a dermal based lesion composed of small irregular duct-like structures in a backgroud of sclerotic stroma.
High power view shows multiple small ducts lined by bland epithelium with prominent cytoplasmic clearing. Some of the ducts are dilated with luminal eosinophilic material.
Clear cell syringoma is considered a variant of syringoma. It is characterized by multiple small dermal ducts lined by a single or double layer of epithelial cells with pale or clear cytoplasm. Like conventional syringoma, the ductal proliferation is bland and often has a comma or tadpole appearance.
The clear cell variant is associated with diabetes mellitus (Furue). It is indistinguishable from conventional syringoma; typically presenting as multiple small papules around the eyelids and cheeks of young women.
Treatment options include surgical excision, dermabrasion, cryotherapy, electrodesiccation, and locally destructive therapy with CO2 laser.
These are benign adnexal neoplasms of eccrine origin. Treatment is generally offered for cosmetic reasons.
